Clinical records created during your treatment are governed by a separate Notice of Privacy Practices that you will receive at your first appointment.
When you submit the reservation form, we collect the name, email address, and phone number you enter. Our server automatically records standard technical information such as your domain name, browser type, and request times for security and reliability purposes; this is not linked to your personally identifying information.
Card data is collected directly by our PCI-compliant third-party payment processor on its own infrastructure. We receive only a transaction reference number. We never see, store, or transmit your card number, CVV, or expiration date.
We use the information you provide solely to process your reservation, schedule your treatment once the clinic opens, contact you about scheduling and your care, and meet our legal and accounting obligations. Information about our customers is an important part of our business, and we are not in the business of selling or renting it.
